As the baby boomers enter into retirement age many of them find that they need assistance managing the basic activities of daily living. The massive numbers of people in this situation have all but over whelmed the system and many are left to deal with it by themselves or rely on family members to help take care of the problems. These companies offer a wide variety of services designed to assist older people with their day to day activities. They are also equipped to take much of the burden of care giving off the family members who traditionally would be doing it. The services are also intended to enable clients to remain in their own homes as long as possible.

An assessment will be taken to gage the needs of the client in question. When an evaluation of the strengths and weaknesses has been made the case plan can be written. There should also be a plan for follow up visits and assessments with no less than six months elapsing between visits. A major goal for the team should be to keep your parent in their own home.

A typical case management plan will contain steps for obtaining services that will take care of cooking, cleaning, doctor visits and shopping. It may also contain a series of visitors that monitor behaviors in an "eyes on" operation should there be a chance of someone taking advantage of your loved one. They will assist with budgeting and writing checks to pay bills or rent. All actions are focused on improving the well being of the customer physically, mentally and socially.

Should the subject be in crisis medically the agencies work with other companies to find temporary or permanent placement for them. Rehabilitation services can be accessed and in some cases the physical therapist will do home visits for the patient. If the placement proves to be permanent they will work closely with family to ensure that the patient gets the very best care available within the boundaries outlined by those involved.

They are also helpful in the realm of final wishes and can help fill out advance directives and medical power of attorney paperwork. Most agents are not allowed to accept roles as executors of these instruments either by company policy or in some cases state laws.

A written plan is usually presented to family members and they can choose to follow it themselves or allow the agency to do it. The agency will follow up with the client in either case. They will also do reassessments periodically and advise the family of any changes needed.
